"Work is still in progress for the "build your own report program" for the website.  In the mean time we have a solution for the following reports:
Resting EKG
Stress Test Report
Spirometry Report
Lab Results

A "summary report" from the physical is being worked on and should be available shortly.

It will take some time before the program mentioned above is up and functioning.

In the interim, if anyone would like access to the above reports from their physical, please email me with specific report requests.  I will then process your request with one of the available options below."

Steps to securely view your physical results:

1.  Send us a request via email stating you wish to see your results from your physical online.  Requesting email address should coincide with the email address we have on file from your visit.  You should include your name, address, last 4 digits ss#, and employer in the email request.  You will receive instructions with name and password to view or download your results anonymously and securely.

2.  Send us a request via email stating you wish to see your results from your physical via fax.  Requesting email address should coincide with the email address we have on file from your visit.  You should include your name, address, last 4 digits ss#, and employer in the email request.  Please be sure to include fax number.

3.  Send us a request via email stating you wish to see your results from your physical via email with attachment.  Requesting email address should coincide with the email address we have on file from your visit.  You should include your name, address, last 4 digits ss#, and employer in the email request.
