If you have reached this page, you probably already know that Dr. Fragen separated from the Advocate Medical Group on May 27, 2005.  Please review the information below to learn how you may be affected.  There is also information regarding some of the services that will be available through the Health Endeavors website.   

PPO Contracts

We are working to convert Dr. Fragen's PPO contracts over to the new business, plus we hope to add a few more. Please visit this page frequently to see the updated list of Accepted PPO Insurance Providers.

Advocate-Lutheran General Health Partners HMO Network

If you are a member of the Advocate-Lutheran General Health Partners HMO network, after obtaining a referral from your HMO PCP, use our scheduling page to set an appointment.

Payments and Scheduling

During the transition period (30-90 days), patient bills must be paid at the time of service.  Appropriate documentation, including diagnosis and procedure codes will be provided for you to submit to your insurance company for reimbursement.  The option to pay for services when provided remains an ongoing option for those of you who do not have insurance or prefer to request reimbursement.  Please use our scheduling page to set an appointment.
